Islay Patient Travel Survey

Throughout May, Islay charity Islay Connections, the Argyll and Bute
Health and Social Care Partnership, the Islay Dental Practice, the Islay
Medical Practice and Islay Hospital will be running a patient travel
survey to gather information about the journeys that Islay residents
have to make for medical and dental care. They want to know about
journeys on the island itself as well as patient journeys to the mainland.
They want to develop a comprehensive understanding of the way that
current arrangements work to see if they can be improved and to
explore whether new voluntary services, such as volunteer drivers and
escorts, could be started.

A PDF to print out at home is available here or you can collect them from all of the GP Surgeries, Dentist or Islay Hospital, where the collection boxes for the completed forms can be found too. Please note the forms are anonymous, can be completed for past experiences and will be processed by Islay Connections.
